ARIZONA CITATION AND COMPLAINT (Form AZ-17-4)
is a four-part citation. (Price
Group 17) The face of all four copies is shown at right.
The back of the first copy ("Complaint Copy") contains
Judgements and Orders of the Court as well as information
about any arraignment. The backs of the second and third copies
are blank. The back of the fourth copy ("Violator / Defender
Copy") contains an Important Notice to Defendant.
The citation is 8.5 inches by 11 inches after being torn
at the perforations and is booked in sets of twenty-five.
Each book is numbered on the outside to aid you in your record
keeping. As with all our tickets, there is no extra charge
for consecutive numbering and up to five lines of crash imprinting.
